Sildenafil and tadalafil get lumped together because they work the same way. Where they differ — how long they last, how food affects them, and how you take them — is what actually decides which one fits your life.
Both are PDE5 inhibitors. They don't create arousal on their own; they make it easier for blood flow to respond when you're already aroused. Sildenafil is the active ingredient in Viagra®; tadalafil is the active ingredient in Cialis®. The generic versions contain the same active ingredient as the brand.
Sildenafil is typically taken about an hour before sexual activity and works for a window of a few hours. Tadalafil lasts much longer — its labeling describes improved erectile function for up to 36 hours — and it's also prescribed as a lower daily dose, which removes the need to plan around a pill at all.
A heavy, high-fat meal can delay how quickly sildenafil starts working. Tadalafil's absorption isn't meaningfully affected by food, which is part of why some men find it easier to live with.
The common ones overlap: headache, flushing, nasal congestion, and indigestion. Sildenafil can briefly tint vision blue or make colors harder to tell apart; tadalafil is more associated with back pain and muscle aches. Get emergency care for an erection lasting more than four hours, or any sudden loss of vision or hearing.
Neither can be combined with nitrates (such as nitroglycerin for chest pain) or riociguat — the combination can drop blood pressure dangerously. Some blood pressure and prostate medications (alpha-blockers) also need careful dosing alongside them. That's why a licensed provider reviews your health history and current medications before either is prescribed.
